ER Nucynta.. what to expect?

Hi guys,

Don't post much, am pretty private but have been a member for some time. 

I recently bought some ER Nucynta from an approved vendor here (international) and after researching more (what I should have done before placing the order) it seems this med is formulated to try and break the 'high' that comes from traditional pain killers and just go for the pain relief.  When I saw that it was listed I just jumped at it as my old pain dr had recommended it when it first came out before this war on drugs but I opted not to use it and just stick with my tramadol (which I now have to get from said vendor also) 

I need something for breakthrough pain and don't anticipate that this will work for that. I have RA and RA related OA which has become quite severe. The strongest med I can get thus far is co-codomol. I'm always scared when trying a new med especially from other countries but b/c I take tramadol wonder if I should make sure I do not take them on the same day as I have read that nucynta has similiar SNRI properties. And b/c it's ER (once again I didn't know when I ordered, if so would have passed) how will I even know if it's working? The tramadol I take is for my depression (if you haven't read the studies they can be found online) so for me I would not be able to take both. How they made it a S2 is beyond me as it seems so innocuous from what I've read. I could be wrong though so am looking for some insight.

Most people I know who are scripted Tramadol use Hydrocodone for breakthrough. I’m certainly not an expert, and this is purely anecdotal. I would find a site where you can check for interactions to Tramadol. This is one I have used. It does state that Tramadol and Nucynta have a major interaction.

Nucynta is like a super Tramadol. I was increased  to Nucynta from 400mg Tramadol (recc maximum amt daily) and it didn’t give me any pain relief...at all. Tramadol is weird stuff because of the SSRI/SNRI effects and it’s a weak opioid.  The serotonin effect acts as a mood elevator. Use Duck Duck Go search engine  vs. google for privacy and search Oregon MME conversion chart ( It’s very user friendly) or just “Morphine Conversion Chart “ to see the difference between Tramadol,Nucynta,Codeine etc.

Nucynta is one of those extremely expensive newer synthetic drugs that disrupt the brains perception of pain ( like gabapentine, topomax, cymbalta and lyrica rather than pure morphine based pain meds. @Gracie5 is absolutely correct that breakthrough pain is best managed by hydrocodeine or a pure opioid agonist. Your depression may be from pain but I’ve not heard it being rx. strictly for depression.

I don’t know if this helps but my Nucynta was not ER and I think it was 50mg bid (x2 a day) or every 12 hrs. Really crap...to put it bluntly !!!

 
Absolutely use GoDuckGo (that’s what I used too). 😊 @Nitetrainis much more knowledgeable than I. I never could use Tramadol, nor Nucynta because my doctor said it was essentially the same drug, only Nucynta was stronger, and if I couldn’t tolerate one, I couldn’t tolerate the other. Again, just like Nitetrain explained well. Just be careful @savingtessthat you don’t use anything too strong, which is why I recommended Hydrocodone. One thing I’ve had a lot of experience with is having my pain meds increased too much, too quickly, and building a very high tolerance. Keep your doses as small as possible.

 
Thank you guys. I wish I had gotten the IR but my vendor didn't carry it yet - says he believes he will be soon. 

As for cutting through acute pain though - which I have a lot of because of my RA/OA since I was young, has anyone crushed them to get a quicker effect? Because of how new this med is in comparison with other drugs I haven't heard much about this at all and am very wary of trying.
