P management doc suggestions in Utah

I have been referred to a PM doc in Northern Utah.  I've jumped through all the hoops my primary care doc asked.  Physical therapy, alternative drugs I.e Motrin, muscle relaxers.  Injections of something.  And reg Tylenol.  I'd like to know if anyone in northern Utah is familiar and recommends a PM doc.  I don't want to go through months of trying garbage when I know what works for me and what is in general the best for me.  I mainly need them so I can get a good nights sleep and so I can still be somewhat active.  I'd like to do this the right way but has proven to be very difficult.  Any suggestions and help is much appreciated.  Thanks

PS. I am only 33 and not looking to do surgery this young.

 
Im in the same boat their ate pain clinics i think you have to get a referral for. I know what works fir me..i know what need to do but i cannot find 1 doc to help me

 
Responding late but may benefit someone. If you are located in the Salt Lake City area; I strongly recommend Omega Pain Clinic. I was a patient with them for 4 yrs but relocated to Texas. Thanks to them, I have lived a pain free life and am able to care for my family and be successful in my career. They also helped me with finding a new pain management doc in Texas. 

 
Any updates to this? I am experiencing chronic back pain - I have a job where I stand all day. Currently without insurance. I am willing to pay cash to visit a doc, but I am not going to jump through hoops. I know what works for me.

Nothing was in the system. It was a huge mess. I had no clue I was signing up for Real ID. They asked me for the exact same documents that I needed for a State ID Card.

  20. B @ bigblueallda: @drjimmy1964 In my state to get a Real ID, I need SS card, Birth Certificate and two proofs of residency. Which I am positive I had to provide to get a new ID when it had been expired.
