When you have genuine pain on a daily basis for long term desiese but can't stick too the meds Daily recommended allowance: what do you do? 

When tapering to low dose won't do because of breakthrough pain yet staying at recommended allowance isn't possible through learnt addictive behaviours.

i hate this catch 22. 

Anyone got a solution?

 
The docs will tell you to just take the meds as prescribed. Sounds simple, but it's virtually impossible some days. I guess my pain doc thinks he's got it solved by pretty much keeping me in pain all the time. It's not his pain, and it's not the D3@'s.

At least their way I tend to take my meds closer to the amount pr3scr!b3d, because I'm nowhere near to being pain-free, and I know that things will be horrible if the end of the pill bottle happens before the end of the month.

Hurt and wd, or stay on schedule. It's draconian and simple, but it isn't easy at all. That's why I'm on this forum.

I totally agree that it's virtually impossible to not run out of meds before it's time for a refill.  My PM doctor changed my script to 2 5mg oxys a day which is 60 a month.  Some days are worse than others so on those days I take more than 2.  The problem is if I tell them that 2 a day is not enough I'm afraid they will take them all away!  In my opinion pain management is a joke.

Worst case scenario they just refund you after checkout. I would just try that if you really need it for medical purposes. My gut is telling me this is what I had to do but like I said, i can't recall 100%
